During her last combat mission across the treacherous terrain of Afghanistan, Royal Air Force pilot Lieutenant Kate Sinclair's jet is brought down near a heavily fortified rebel stronghold. As the insurgents pursue her at the crash site, she seeks shelter in an abandoned underground bunker. While tracking her through the depths of the facility, the militants unknowingly activate a man-made biological weapon—Ravagers, synthetic creatures part human and part alien, that crave human flesh. Horrified, she witnesses the creatures tearing apart the soldiers in pursuit. Sinclair barely survives and is later rescued by a US army patrol deployed to locate her. Major Roy Finch, the leader of the local outpost 'Fort Apache,' and his eclectic team of soldiers struggle to believe that such monstrous entities have been concealed in these mountains. Unaware of her presence, a swarm of the vicious Ravagers is already en route to their position.
